Submit a Provider Alert

Provider Alerts can be sent via email by Falling Colors on behalf of member agencies with important news for providers participating in agency projects/receiving agency funding.

Provider Alerts are then posted here: Prior Provider Alerts for provider reference.

Alerts should be submitted to Falling Colors on the specified Provider Alert template detailing the contents, the date for sending, and the intended audience.

Member Agencies should follow their own Agency guidelines for review and approval with final approval through the Agency. The Agency will notify the requesting Member Agency of approval or corrections needed. The Agency shall be the only submitter of Provider Alerts to Falling Colors.
