Triage and Assign an Incident
Section Summary: The Triage user at the Lead Agency views the report for completeness and to determine if it should be reported to their Lead Agency. If the report is incomplete, they can return it to the originator or reviewer, adding comments indicating the edits required (These comments are viewable to Originator, Reviewer(s), and all Oversight users). If the report should not be reviewed by their agency, the triage user can make that distinction giving oversight responsibility to the appropriate agency. If the report is complete and ready for investigation, they can assign Flags and Acuity, assign an Investigator, and send notification about the submitted report.
As a Triage user at the State Agency, when new reports are submitted and ready for Triage, you will receive a notification in email, text message, or both, depending on the preferences you have set.
Step 1 - Find & Open the Report
If you receive a notification that a new report needs attention, when you sign in to the system, you can easily find that report on the Incidents page by selecting the Originating Organization:
Or, if needed, you can also filter the table to reports in the Awaiting State Review status and then click the Open icon on the report.
You can navigate through the Incident Report sections using the menu on the left.
Step 2 - Add Comments to the Report
The comments interface includes comments added to the report by any users, including the Originator, any Reviewers, and any LCA Triage or Investigative users. You can toggle between viewing just the comments added on this question, or all comments for this report.
Click the triangle icon to submit comments. As you add your comments throughout the report, and the application will keep track of where each comment was added.
You can click the Trashcan icon to remove a submitted comment. You can also click the Open icon beside any comment to jump to the question where that comment was added.
Step 3 - Complete the Triage Review
When you have reviewed the entire report and finished adding comments, click the Finalize tab.
Step 4 - Submit the Review Outcome
Finally, select the Triage outcome:
Accept - If you determine that this Incident Report is complete and of sufficient quality.
Return to Originator - If you determined that the report needs revisions.
Return to Managed Location and Remove Skip Review Status:
The report will be set to Awaiting Provider Review status.
The report will become visible to all Reviewers at the Managed Location, as well as remain visible to the Originator and everyone at the LCA.
All Reviewers at the Managed Location will receive a notification that the report has been returned for additional information.
Any Reviewer at the Managed Location can edit the report and either return it to the Originator or re-submit the report to the LCA (“Submit to Oversight”).
Then click the Complete button.
Step 5 - Do It Again with Resubmitted Reports
Reports that are Returned to the Managed Organization with comments requesting revisions will generate notifications to the assigned Reviewer of the report. The Reviewer can then request further revisions of the report originator, if needed, or make their own revisions and resubmit for Triage.
When those revisions have been made, and the report is resubmitted, you will be able to jump directly to the questions where you requested revisions and determine whether the updates submitted are satisfactory.
Step 6 - Set the Report Acuity and Assign it to an Agent
Reports that are Accepted after Triage review need to have the Acuity and Agent assigned.
Click the Acuity drop-down on the Incidents page and click the appropriate Acuity.
Click the Responsible Agent drop-down on the Incidents page and click the appropriate Agent.
