Revise an Incident
As a Staff or Reviewer-type user at a Provider, reports you’ve submitted may be returned with comments requesting revisions. When this occurs, you will receive a notification in email, text message, or both, depending on the preferences you have set.
Step 1 - Find & Open the Report
If you receive a notification that a report you’ve submitted needs attention, when you sign in to the system, you can easily find that report on the Incidents page and click the open icon.
Or, to narrow further, the filter field applies across any of the columns you see. For example, to find reports that have been returned for revision, try filtering for “Returned”. Then click the Open icon.
Step 2 - Assess the Comments for Requested Changes
When revising a returned report, the Comments on the right side of the screen will be your guide.
Click “All” to see any comments added by any users throughout the entire report. When you find a comment that requires your attention, you can click the Open icon beside the comment to jump to the appropriate question.
Step 3 - Make the Requested Changes
In the example below, the commenter asked that the Originator add client details.
Checking the box for “Clients were involved in or witnessed the incident” adds a Clients Involved section to the report, which can then be completed as requested.
Step 4 - Add Comments
After making any requested changes, it is helpful to respond to the comments to describe the changes you made.
After you click the triangle icon to send your comments, you can click the trashcan icon to remove a submitted comment until you click Complete, at which point your comments are saved permanently.
Step 5 - Complete Your Revisions
When you’ve made all the requested changes, click the Finalize tab.
Step 6 - Choose Your Submission Method
Abandon: If you determine that this Incident is not actually reportable (this option is only available if this report has not yet been previously submitted to the State Agency for Triage).
Submit for Review: If this is a typical report, which should be looked over by a Reviewer.
Skip Review: This option should ONLY be selected in special circumstances, for example:
Submission of an urgent report occurs at night when no reviewer will be available at the provider location.
The report is exceptionally urgent and needs to reach the State Agency immediately.
I believe I am a whistleblower and would like to submit the report anonymously, without review by supervisors.
