Edit a Plan of Safe Care
A Plan of Safe Care can be edited while it is in a “Pending” Status by the Hospital creator of the plan and before it has been submitted to an MCO. Care Coordinators cannot edit a Plan of Safe Care.
Step 1
Locate the Plan of Safe Care on your Plans of Care list and click the Edit Icon.
Step 2
Resume filling out all sections of the Plan of Safe Care.
Tips & Tricks
As you fill out the Plan of Safe Care details, you can move sequentially through questions with the Previous & Next buttons or jump between questions using the navigation bar on the left.
There will be checkmarks indicating completed questions, and the counters indicating how much of each section and the entire document you’ve completed as you proceed.
Required fields are indicated with an *.
Tips & Tricks
You can collapse or expand sections to quickly navigate through the Plan of Safe Care.
Warning indicators (stop sign icons) will appear if your answers to any questions are incomplete or incorrectly formatted.
These indicators will appear beside the question, and at the top of any section containing questions needing attention.
Based on your answers to some questions, you will see additional questions appear, for example, based on the number of key household members indicated, you will be asked to fill out additional questions for each.
Save and Exit and an Exit Icons will be displayed in the lower right so that a Plan of Safe Care can be begun and saved, and then re-accessed later for completion.
A “Complete” button is available, but is only available to be clicked when all required fields in the survey have been filled in.
More Information Needed
Sometimes the Plan of Safe Care will require additional information, based on an answer you provide. For example, in the Support Services section, you’ll need to provide details about any services marked “Yes” as shown below:
Step 3
When all the fields have been filled out correctly, your form will be saved, and the “Complete” option will appear at the bottom right of the question card. Click “Complete” to complete the Plan of Safe Care.
Step 4
The system will return you to the Plans of Care screen and generate a PDF of the completed Plan of Safe Care. To view and print the Plan of Safe Care, locate the Plan of Safe Care you just created and click the Download Icon.
Step 5
When you have reviewed the Plan of Safe Care and are ready to submit, click the Submit Icon. Select the MCO, enter any free text Additional Comments, and click Submit. The MCO Manager will receive an email notification.
