Please refer to the links on the left for more info on using the new portal, with features such as:

•  The ability for a user to start a plan of care, save and exit, and edit for completion later. 

• The ability for a user to abandon (delete) an unsubmitted plan of care. 

• All Plans of Care must now be submitted after completion to an MCO or, if MCO is unknown, to CMS. This will support the automatic transfer of Plans of Care from hospitals to the State and to the relevant MCO.  

• New and updated fields in the plan of care, including new services, optional fields, and tracking when a family declines the plan of care. 

• Additional Roles have been added for: SCI, MCOs, Care Coordinators, and State Oversight. 

• Email Notifications-state staff and appropriate MCO managers will get an email when a plan is submitted, and Care Coordinators will get an email when a Plan is assigned to them.
