Add a Location/Site to NMSTAR

To add a new location/site to your provider in NMSTAR, please send a request to us at support@fallingcolors.com with the following information:

• Location/Site Name

• Location/Site NPI

• Vendor Administrator

Falling Colors Support will:

• establish the site in NMSTAR and link NPI

• grant the designated Vendor Administrator access to complete the profile in NMSTAR

The Vendor Administrator will:

• establish an account in NMSTAR

• log in to NMSTAR

• add the required information (address, phone #, contacts, etc.)

• ensure the profile is accurate and complete

• submit the profile for approval

• let us know when the site has been submitted.

Falling Colors Support will then review the location/site and respond to the request to let you know:

• profile has been approved and they can move forward with this new location/site in NMSTAR o

• if additional information is required.
