Participant Registration

From the Home screen:

1. Click Client, Register.

2. Click the First Name field and enter name.

3. Click or Tab to the Last Name field and enter name.

4. Click or Tab to the Date of Birth Field and enter DOB.

5. Click Search.

If the participant is not found, the system displays a message-Click Create New Record.

If participants with that same Name and DOB are found, the system will display similar records-Click a Participant ID if it is that participant, or Click None of These to create a new record.

6. Click the Preferred Name field and enter the participant’s preferred name.

7. Click or Tab to the Email field and enter the participant’s email address.

8. Click or Tab to the SSN field and enter the participant’s Social Security Number.

9. Click or Tab to the ESM ID field and enter the participant's unique ESM ID. (The ESM ID field label is a link you can click to access and log into their Virtual Gateay)

10. Click the Insurance Type field and click insurance.

11. Click or Tab to the Insurance ID field and enter the identification # for the participant’s insurance.

12. Click or Tab to the Gender field and select the participant’s gender.

13. Click or Tab to the LGBTQ field and select the participant’s status.

14. Click or Tab to the Language field and select the participant’s primary spoken language. (Note: If Other is selected the system will then require a free text entry in the Other Language field)

15. Click or Tab to the Military Service field select the participant’s current/past military status.

16. Click or Tab to the Ethnicity field select the participant’s Ethnicity.

17. Click or Tab to the Race field and select all that apply. (Note: If Native American is selected the system will then require a selection in the Tribal Affiliation Status field and if Other is selected, a free text entry in the Other Tribal Affiliation field is required.)

18. Click Save and Continue. If any required fields are missing the system will display Required in that field to indicate it must be entered or selected to proceed.

19. Click Address Line 1 field and enter the participant’s home address or Click the Checkbox to indicate the participant is homeless.

20. Click or Tab to the Address Line field and enter if needed.

21. Click or Tab to the City field and enter the participant's city.

22. Click to Tab to the State field and select the participant's state.

23. Click or Tab to the Zip Code field and enter the participant's zip code.

24. Click or Tab to the Primary Phone field and enter the participant's phone number or Click the Checkbox to indicate the participant has no phone.

25. Click or Tab to the Extension field and enter the participant’s extension if needed.

26. Click or Tab to the Text OK? field and enter whether the client will accept text messages at this number.

27. Click or Tab to the Voicemail OK? field and select to indicate if a voicemail can be left at this number.

28. Click or Tab to the Other Phone and enter if needed.

29. Click or Tab to the Message field and select to indicate if a message can be left at this number if needed.

30. Click or Tab to the Extension field and enter the participant’s extension if needed.

31. Click or Tab to the Text OK? field and enter whether the client will accept text messages at this number.

32. Click Save and Continue.

Not all fields on the Contact screen are required-However, at least 3 Collateral Contacts per participant is required.

33. Click the Name field under a Contact and enter a name.

34. Click the Phone Number field under a Contact and enter a phone number.

35. Click the Email field under a Contact and enter email address. (If the contact is associated to an Organization-Enter the name of the Organization and their Role.)

36. Click the Relation to Client drop-down under a Contact and select a relationship status.

37. Click Save and Continue.

38. Click the Project drop-down and click the ATR Project.

39. Click the Grant drop-down and click the grant.

40. Click the Coordinator Location drop-down and click the participant’s coordinator location.

41. Click the Coordinator drop-down and click the participant’s coordinator.

42. Click the Referral Provider drop-down and click the provider.

43. Click the Referral Program drop-down and click the program.

44. Click or Tab to the Enrollment Date field and enter the participant’s date of enrollment for ATR. (Note: The system will auto-calculate and display the Disenrollment Date and the Budget)

45. Click Save and Continue. (Note: The system will auto-calculate the Days Remaining in the Program)
