Click the Program from the left navigation.
Click the Surveys tab for the appropriate project.
The survey card will display listing any completed surveys along with the Date and Administered by.
Click the triple dot icon to Copy a survey forward or to edit an uncompleted survey.
Click the Start New button to start a new survey.
The survey will be displayed.
Fill in all required fields (Designated with a * icon).
Click the Previous and Next to navigate survey pages.
Click Save and Exit to save an uncomplete survey and come back later to complete.
OR
Click the Complete Survey to save a completed survey-The button will only appear active when all required fields have been answered.
