Step 1
On the New Mexico Healthy Families Home page, click the download icon at the top-right of the screen, and click Follow Up. A PDF of the Plan of Care Follow-up Priorities Form will download.
Step 2
Locate the file in your browser’s download folder, and open or print the file as needed.
