Clients with Suicide Risk

From the Home screen:

  1. Click ‘Clients with Suicide Risk’.

  2. Click the Client ID of the individual found on the list of clients at risk.

  3. The Client Dashboard is displayed.

Client Dashboard

The Client Dashboard can be accessed via the ‘Clients with Suicide Risk’ tile or Client Find. If there is a risk of suicide for the client, an orange banner will appear at the top of the Client Dashboard.

  1. Click on the Services tab.

  2. A list of CareLink services performed on the client is displayed, if any.

Tip: The Services tab will only appear if the client is activated in the CareLink project.

  1. Click on the CNA tab.

  2. Click the blue Flags button to see all the active, archived, and historic flags for that client.

  3. Click the blue View Care Plan button to view the client’s Care Plan.

  4. Click the blue View CNA button to view the client’s CNA.

  5. If a Clinical Screen has been performed on the client, click the teal View link next to the date of the Clinical Screen.

  6. If a C-SSRS survey has been performed on the client, click the teal Table link in the box of the C-SSRS survey, then click the View link next to the date of the C-SSRS survey.

  7. If an Anxiety survey has been performed on the client, click the teal Table link in the box of the Anxiety survey, then click the teal View link next to the date of the Anxiety survey.

  8. If a Depression survey has been performed on the client, click the teal Table link in the box of the Depression survey, then click the teal View link next to the date of the Depression survey.

  9. If an Audit-10 survey has been performed on the client, click the teal Table link in the box of the Audit-10 survey, then click the teal View link next to the date of the Audit-10 survey.

  10. If a PC-PTSD survey has been performed on the client, click the teal Table link in the box of the PC-PTSD survey, then click the teal View link next to the date of the PC-PTSD survey.

Tip: The CNA tab will only appear if the client is activated in the CareLink project.

  1. Click the Projects tab.

  2. If the client is activated in the CareLink project, the date of Activation is displayed.

  3. If the client is activated in the CareLink project, the Singing and Effective date of Opt-in or Opt-out are displayed.

Client Contact Information

  1. Click Profile or Address & Phone tab.

Tip: None of the fields can be edited & saved.  Only providers can apply changes to the Profile and Address & Phone tabs.

Client Mandated Data

  1. Click the Mandated Data tab.

  2. Click the teal link next to any Mandated Data records (Initial Registration, Client Check-in, Client Deactivation, and/or Reactivation), if any.

Reports

From the Home screen:

  1. Click Reports drop-down and click report.

  2. Click Provider Site drop-down and click site or click the calendar and click dates.

  3. Click Run Report.

  4. Report details are displayed.

 

 

Tip: To see actual clients receiving the service, click the Service Count.

Tip: To change the search criteria re-click the search criteria fields and re-click run report.
