The Attestation is an annual requirement for all Behavioral Health providers, and attests to the credentialing of staff at the organization.  The form requires Providers to indicate yes or no if the organization is credentialed by either of New Mexico’s MCOs.  The annual attestation is sent to each new Provider that registers in STAR + Contracts with Falling Colors, and each year annually with the fiscal year change over (normally mid to late July).

Note: FCC will send reminders to Providers to complete the attestations each week for a month. At the end of that month, FCC will send a list of Providers with outstanding Attestations for the State’s review and guidance on next steps.
