Incident Reports have different statuses that change as they proceed thru the process.

Status

Definition

Incomplete

A report that has been created but not yet submitted.

Awaiting Review

A report that is waiting for a Provider Reviewer to open and review for completeness and quality, add comments, return to the originator for clarifications, or submit to LCA.

Returned to Originator

A report that needs to be reviewed for completeness and quality based on comments from the Provider Reviewer or LCA, and then resubmitted to the Provider Reviewer or LCA.

Awaiting Triage

A report that has been submitted and is awaiting review by LCA who adds comments to the report, then either returns the report to the Provider for revisions, or Accepts the report.

Accepted

A report that has been submitted and accepted by LCA which is awaiting assignment to an LCA Agent for investigation.

Investigating

A report that is currently under investigation by an LCA Agent.

Closed

A report that has been closed by an LCA Agent.

Agency Review

A report that has been identified as needing Agency Review.

Corrective Action

A report that has been identified as needing Corrective Action.

Abandoned

A report that has been determined to be not necessary or no longer requires action and investigation.
