The following documents provide the requirements for what constitutes a Critical Incident Report and therefore should be reported to the State Agency.

For BHSD:

SENTINEL EVENT:  An occurrence that meets one or both of the following conditions:

(1) Involving serious physical or psychological injury or the risk thereof and requiring monitoring or transfer to a higher level of care, or

(2) Resulting in death.
